You must install OpenIV first, then with OpenIV, go to GTAIV\pc\models\cdimages\vehicles.img and enable edit mode after that drag-n-drop dukes.wft and dukes.wtd to inside vehicles.img.
Also to install add-on cars,
You must first get carcols.dat, handling.dat, vehicles.ide and vehOff.csv lines and copy each line into their files. These files (carcols.dat, handling.dat, vehicles.ide and vehOff.csv) are in GTAIV\common\data.

Looks fine but why plate is custom? Why there isn't an option to make it classic Liberty City plate?
